New Patients

If you are living within our practice area, you can register with us by picking up a registration pack from either surgery, completing the registration forms and then returning them to us.
The registration pack contains a registration form, information about the practice and a health questionnaire. If you need assistance with the health questionnaire, the reception staff will be able to check your blood pressure, height and weight and check a urine sample for you.

You will be encouraged to have a consultation with either a GP or the Practice Nurse within the first six months of registering to enable us to assess your general health. If you take regular prescribed medication you will need to see a doctor before we can start giving you this.
